* fixed mutex initialization bug
just some refactoring work: - renamed class LinuxSamplerException -> Exception - encapsulated LS API relevant files into LS namespace - removed unnecessary header inclusions
* added linear interpolation of volume modulation inside a subfragment; this prevents clicks during voice stealing. Can be switched off with the --disable-interpolate-volume configure option.
